    Default Most excellent series on how we fail . . .

    In answer to a question of how to avoid the pitfalls of diving beyond training / experience, I was referred to this Four Part series on "Normalization of Deviance". If you work or play in a team, this is a premiere seminar on how standards slide, and how to recognize that and stop it.

    Basically, my take-away from the series, as pertains to diving, is that every now and then, you should ask someone not in your circle to do an assessment - where have I developed complacency? Took a short cut? Where did I lower standards such that I now have a new, lower, standard?

    I've learned from other divers how they did first one, very short, visual jump; just popped into this room and will come right back out . . . Then the story of a near miss or a fatality makes them take stock in what they are doing and they resolve not to do it again.

    I'm trying to learn how to recognize those first baby steps on the slippery slope . . . before I screw up and make them.

    I really liked these videos because you can SO relate them to "Stupid Diving Tricks".

    What I came up with, for example:

    -- I've reached my rule of sixths, but maybe I can stretch it a 100psi or so . . .

    -- I'm not trained to go deeper than 100fsw, but I could do a quick bounce dive . . .

    -- I've reached my gas plan turn-around, but given a little longer, I can get this picture . . .

    -- I know the MOD is 100fsw, but it won't hurt to go just a little deeper because the 1.4Bar is such a conservative number . . .

    -- Wow, the current is really strong . . . but I can do this by grabbing and pulling myself on the rocks coming back . . .

    -- Screw the safety stop, it's just an opinion . . .

    Any other ideas?

    Not Dive related . . . but IMO they're still making "Criticality One Deviations" on Solid Rocket Booster Launches.

    From last year at Vandenberg AFB and the heaviest payload in history ever to launch into orbit from the West Coast: the O-rings on the solid fuel rocket boosters "let-go" and leaked fire for a few seconds --a la the Challenger Disaster. (You can see the fire & smoke trail near the top quarter of the boosters at 0:56 to 1:22; and at 1:36 in the video link below). . .
